Responsibilities include providing case management for 35-45 individuals, including eligibility determination, management of the waiting list, authorization and purchase of services and supports, service and support coordination, monitoring, implementation of safeguards to prevent conflict of interest between case management and direct service provision, and termination and discharge. Preferred qualifications: Bachelor’s Degree in human services or related field and 5 years of experience in developmental disabilities field.
